You can book, re-schedule, or cancel your appointments online up to 48 hours before your scheduled appointment time. Simply use the ‘Book’ button.
Please note that we require 48 hours notice for cancellations.
Depending on your appointment type, you may be sent a secure link to our patient forms (i.e. intake forms, outcome measures) that we ask you to submit no later than 2 business days prior to your appointment.
INTAKE APPOINTMENT: Once your appointment has been confirmed, you will receive a secure link to our patient intake forms that we require you to submit no later than 2 business days before your appointment.
COVID-19 SAFETY PROTOCOLS:
Please complete the Ontario Ministry of Health’s COVID-19 Screening Questionnaire 1 day prior to your scheduled Appointment and notify us of any changes in the interim. This is a mandatory requirement for anyone presenting to the office.
WE ALSO REQUIRE PATIENTS TO WEAR A WELL-FITTED 3-PLY SURGICAL MASK OR EQUIVALENT (COVERING THE NOSE AND CHIN) WHILE IN THE OFFICE/BUILDING.
NEW: THE CSI SPACE WHERE OUR OFFICE IS LOCATED REQUIRES EVERY TENANT AND GUEST TO BE FULLY VACCINATED IN ORDER TO BE ALLOWED TO ENTER THE PREMISES!
DIGITAL FOOT ASSESSMENT (FREE)
We offer a complimentary Digital Foot Assessment to find out whether
- your feet could be contributing to your lower extremity or spinal pain
- you are prone to overuse injuries
- you could benefit from custom orthotics
We use cutting edge digital technology to objectively pre-determine your custom orthotics need and provide you with an instantaneous report.
We are also happy to discuss next steps and tell you more about our services.

Orthotics INTAKE APPOINTMENT
(telehealth)
In order to reduce your time in the clinic to provide you with the safest and most efficient experience, part of your intake appointment (e.g. taking your health history, assessing pertinent lifestyle factors) may be conducted over the phone (prior to your in-office appointment).
We may suggest this option to you upon review of your intake forms.

Orthotics intake appointment
(IN-OFFICE)
Your In-Office Intake Appointment consists of the following:
- History Taking
- Biomechanical Assessment + Gait Analysis
- Footwear Screening
- Report of Findings (incl. Diagnosis + Plan of Management)
- Orthotics Casting
Your custom orthotics will be ordered from the lab(s) and full payment is required at this time (orthotics fees range from $550-$600 per pair).
Many extended health benefit plans cover custom orthotics. You will be issued a receipt that you can submit to your insurance provider (if applicable).

orthotic fitting
Your orthotic fitting is usually scheduled within two weeks of your intake appointment.
During this appointment, your shoes will be fitted with your custom orthotics, and we will provide you with instructions regarding proper wear and care of your orthotics.
Please bring all shoes to be fitted with your new orthotics.
Since custom orthotics work best in proper footwear, we will also do a footwear audit with you. This will give you the knowledge you need when assessing your footwear at home and when shopping for new footwear.
Before we let you go, we may briefly re-assess your posture, gait, and select functional movements while you are wearing your new orthotics.
oRTHOTIC follow-up appointment
To ensure comfort and compliance of patients fitted with orthotics, a clinical follow-up will be scheduled (usually within 90 days of your fitting appointment) and any required adjustments requested from the lab.
We may also perform a brief re-evaluation to assess your progress and satisfaction thus far.
annual check-up (Orthotics)
We recommend scheduling a check-up every year to ensure your orthotic support is still adequate for your needs.
If you have extended health coverage and are eligible for a pair of orthotics each year, we recommend scheduling your appointment 2 months before the end of your insurance year.
2-year recall (orthotics)
It is recommended to replace regularly worn custom orthotics about every two years.
Although your orthotics may last longer if you rotate multiple pairs of orthotics, we are confident that you can’t wait to add another pair or two to your line-up.
A lot can change in two years, including your weight and health history.
We recommend scheduling a recall appointment every two years to check and replace orthotics older than 2 years.

footwear audit (FREE)
Have your favourite footwear checked for correct size & wear pattern and your feet scanned to determine whether your shoes are suitable for your foot shape/type
This service is always free for our orthotics patients. We extend this offer to the community during Footwear Awareness Month (March).
orthopaedic/ diabetic shoe fitting (first visit)
On your first visit, measurements (manual, digital, and visual) will be taken to determine your correct shoe size, foot shape, arch height, and other pertinent factors upon which our footwear recommendations for you will be based.
We offer off-the-shelf/stock orthopaedic shoes from Apex and Anodyne as well as shoe modifications.
Your footwear will be ordered, and full payment is due at this time (off-the-shelf/stock footwear options range from $185-$270).
Once we receive your shoes, we will schedule you for a fitting (subsequent visit).
Some extended health benefit plans cover off-the-shelf/stock orthopaedic/diabetic shoes and you may be eligible for a new pair each year.

Once we receive your shoes, you may schedule your fitting.
Please wear the socks that you are planning on wearing with your new shoes and bring your custom orthotics (if applicable) with you.
POSTURE CHECK
A Posture Check is a visual and digital screening of your posture in order to identify postural distortion patterns and establish whether you have a need for therapeutic intervention.
Your Posture Check includes the following:
- Visual Posture Screen
- Posture Imaging
- Digital Foot Assessment
- Report of Findings

fall prevention program (visit 1)
Your first visit consists of the following:
- Patient Intake
- Multifactorial Fall Risk Assessment
This visit is all about the diagnostics. We will review your intake forms, take your medical history, and conduct a physical exam (incl. gait, strength, balance, and vision testing) in order to identify your fall risk factors and determine your fall risk.
We recommend to dress comfortably and be prepared to take off your shoes and socks.
Please plan up to 60 minutes for your visit.
This program is not covered by OHIP but by most extended health benefit plans that pay for chiropractic services. You will be issued a receipt that you can submit to your insurance provider for reimbursement (if applicable).
The program fee is $650 (incl. all materials and handouts) and is to be paid in 3 installments (i.e over the course of your 3 visits).
Our annual Fall Prevention Program opens up once a year in November for new patients.
fall prevention program (visit 2)
Your second visit consists of the following:
- Report of Findings
- Fall Prevention Strategies & Emergency Preparedness (Interactive Education Session incl. Handouts)
- Emergency Preparedness (One-On-One Practice Session): How to get up from a fall
- Home Exercise Program (DVD Set)
During this visit, the results of your fall risk assessment will be shared with you (Report of Findings) and fall prevention and emergency strategies will be discussed and practiced. If this is your first Fall Prevention Program at Urban Soles, you will also be given a progressive Home Exercise Program.
We recommend to dress comfortably and be prepared to go through a series of movements on the floor.
Please plan up to 90 minutes for your visit.
fall prevention program – follow-up (Telehealth Appointment)
During this follow-up, your care plan will be reviewed, fall risk reduction behaviours assessed and encouraged, and barriers to adherence addressed.
metascan™ digital gait assessment
Metascan™ is an advanced digital system for scientific, accurate, and biomechanical patient assessments. It enables the clinician to assess your gait with greater accuracy than what the human eye can perceive.
It is an objective way to pre-dermine whether patients could benefit from custom orthotics.
We use this technology with our corporate clients for on-site visits and make it available on demand for individual clients.
